Patient records

Geriatric Vision Care maintains a complete record of all patients. A detailed log sheet outlines the following information for each visit:

  • Patient name, primary diagnosis, and information related the exam.

  • If they will be receiving new glasses and what type.

  • If an ophthalmologist consult is recommended by the optometrist.

  • The primary diagnosis of the doctor concerning any vision problems.

You will receive a computer log within 30 days after exams. This just lists patient names alphabetically, whether or not they will be getting new glasses, or a notation if an ophthalmological consult is recommended.

Patient billing

Patients are billed any remaining balance due after their primary and secondary insurance have been submitted.

Residents who are Medicaid are not billed for any charges unless authorized due to services out of the scope of their insurance.
